Individual Treatment Plan

Your child's personalized roadmap — goals, strategies, and measurable outcomes, built with your family.

What is an ITP?

An Individual Treatment Plan (ITP) is the personalized roadmap for your child's services. It translates the evaluation into specific, measurable goals and the strategies the team will use to reach them.

The plan is built together with your family, guides every session, and is reviewed and updated regularly as your child grows and progresses.

Who it's for

  • Every child receiving EIDBI or CTSS services
  • Families who want a clear, shared plan for their child's progress

How it works

Start from the evaluation

Goals are grounded in the CMDE or assessment.

Set measurable goals

Guide each session

The plan directs day-to-day services.
